HMRC form FD9

Has anybody had recent experience in getting the form FD9 completed at the Spanish Tax office for return to HMRC. They just don't seem to want to do it down here in Ayamonte?

Thank you for the link to the form at HMRC morerosado it was useful since the form I brought with me from UK now appears to be slightly out of date insofar as the return address has changed from Nottingham to Ty Glass. It is not the form per se but the Hacienda seem very reluctant to accept and sign and seal it.
