Which form ... ?

Last year we used a law firm to file our Renta tax, this year the form 210 is online and we want to file it ourselves without using a representative.  However, we were told we must submit a 2nd form to the Spanish tax office informing them that we no longer use a representative, but are filing our own tax form.  What form number is this?  Where do I look in the Agencia Tributaria site to find it, i.e., what is it called?

You need Domicilio Fiscal ( Modelo 030)  it,s in one of the boxes on the right when you access non residents -en portal on the website.

Go to Cambio de Domicilio;Model 030 Formato PDF then Descarga del Modelo

You then want box 105

Consignación/modificación/cambio de domicilio notificaciones.

You can fill it in and print it out to present at the tax office
