Tax Form 210

No doubt this suject has been well discussed but I am not able to find all the info that I want to complete the form. For a number of years I have completed Form 214  (non-resident) without much difficulty, but this is the first 210 and I'm not sure. Can anyone help me and give me a sample completed form (obviously using ficticiuos details). By the way, we collected the forms from Hacienda this morning and the clerk confirmed that the form has to be returned and taxed paid by the end of December (not 30th June, as we were lead to believe). Any help would be appreciated, but as a matter of ugency. John

I've not been to Spain all year.

Fortunately there is this chap does the form 210 for me and makes the payment to the bank all for a modest sum.
