English version of Modalo 210 ... How do you get an original blank TAX from

Hello ..

NON RESIDENT TAX..... DECLARED and pay

Im sure this has come up before as topic but last year we completed an non resident tax form ( through Spanish Tax uk) and took it to the bank in Spain and paid the tax which was a small amount. This year we will be doing a small amount of Holiday Rentals and have to pay 22% on any income . So is a simple system available  for us to get a completable FORM 210 (Non Resident Tax Form) to pay the Hacienda via our Bank when we are in Spain next ( I understand it has carbon copies) so we can fill it in and Pay tax on a small rental income. ?? We dont want to keep payings the Agents fees to produce the forms in future
